If you would like to send your application via email, take your completed application with your hand written signature to a local ups store to scan document. You can not take a photo of this application to send. It has to be in a pdf form with handwritten signature to be accepted. UPS can scan (request it be in a PDF format) and email you the scanned version.
In the subject line enter confirm receipt with your case number found on the peak website or any mail you have received from Health First Colorado. If you do not have a case number, you should apply for Medicaid first online at health first Colorado or at your local DHS department.
Then you can email the file to cmapbuyin@dhha.org
